As soon as you send the packet back to us, we will be glad to set up an appointment. We want to give you our undivided attention and will attempt to get records prior to the visit. Your first appointment should be available within 1-2 weeks. Please call our friendly office staff if you need help with the forms, need an urgent prescription refill or have a question.
How to save the forms
